In Vancouver, the Fletch Family Daycare, has since been shut down and will remain so until the bacteria is controlled.

It is possible that the failure of employees to properly wash their hands after changing diapers or toilet use could have caused it to spread.

There is an ongoing investigation into the cause of the outbreak.

Two children were killed and a third was seriously injured when a van struck a group of people on a sidewalk in New York City’s busy Chinatown Thursday morning, said police.

Police say the two children were standing among a group of 14 children who were walking back from a library to a day-care center in Manhattan’s Chinatown commercial district shortly before noon Thursday.

A gray of silver Chevrolet van swerved out of control at 11:31 a.m. Tuesday morning after making a U-turn at 38 East Broadway, according to witnesses of the accident and police. The van, from the China Chalet at 47 Broadway, skipped over a curb and struck at least five children on the sidewalk. 14 of the children were treated for minor injuries at Bellevue Hospital, according to spokesman David Zackin.

There are conflicting reports on how the accident occurred.

The van driver told authorities that before the accident occurred he had made a U-turn on the street and then pulled over to make a delivery. When the driver exited the van the engine was still running and it continued to move, plowing into the group.

Police later said they believed that the van was backing up when it jumped the curb and hit the group.

After the accident, the driver, who wore a black hooded jacket, was escorted by police to a police car and taken away from the scene, witnesses said. The driver was being questioned by police.

A young boy was fatally hit with flying metal debris at a monster truck show in Tacoma, Washington on Friday, said officials.

A red truck was doing doughnuts and driving in tight circles when debris flew 30 to 50 feet over a safety barrier and into the stands, said witnesses.

“Parts were falling off and a piece flew up and hit a little boy,” said Christine Moe of Seattle. “You just saw the mother just sitting there holding her son and the whole top of his head was just gushing with blood.”

Another man, who remains unidentified by authorities, was hit with the debris and was “laying on his side, kicking his legs,” said witness Laurie Deranleau.

The injured two were taken to the hospital, where the 6-year-old boy was pronounced dead. He was identified by the Pierce County medical examiner’s office as Sabastian Hizey of Puyallup.

Sebastian had suffered severe brain trauma when he was hit by a 7- to-12-pound, Frisbee-sized metal ring from a truck, said a statement issued by his family.

The remaining four Monster Jam shows, two each on Saturday and Sunday, remained on the Tacoma Dome schedule. “They didn’t even stop the damn show,” Moe said. “They just kept going. We grabbed our kids and just beelined out of there.”

“Our company follows strict safety guidelines designed to protect both spectators and competitors alike,” said Bill Easterly, Feld’s senior director of operations, in a statement issued Friday night. The truck involved was taken out of use while the company investigates, he said.

Kellogg Co. asked stores to stop selling its peanut butter sandwich crackers until it can be found out if the peanut paste is contaminated with salmonella.

The national salmonella outbreak has sickened over 430 people in 43 states and may have been a contributing factor in five deaths.

Lynchburg, Va.-based Peanut Corp. of America, where Kellogg gets some of its paste, has recalled 21 lots of peanut butter made since July 1 at its plant in Blakely, Ga., because of possible contamination from the bacteria.

Kellogg has asked stores nationwide to remove the crackers sold under the Austin and Keebler brand name, known as a “stop-sale” and is not as serious as a recall, and urges consumers to not eat those products until the investigation into Peanut Corp. has been completed.

Kellog is not going so far as issuing a recall, as Kellogg said it hasn’t found problems or received complaints about the products. “We are taking these voluntary actions out of an abundance of caution,” said Kellogg CEO David Mackay.

The products being removed include Austin and Keebler toasted peanut butter sandwich crackers, peanut butter and jelly sandwich crackers, cheese and peanut butter sandwich crackers, and peanut butter-chocolate sandwich crackers.

Health officials in Minnesota and Virginia have both linked two deaths to the outbreak and Idaho has reported one. All five were adults who had salmonella when they died, though their causes of death haven’t been determined. The U.S. Centers for Disease Control and Prevention said the recent outbreak may have been a contributing factor.

There are about 2,000 types of salmonella, the nation’s leading cause of food poisoning. About 40,000 cases are reported each year.

CDC officials say the bacteria in this outbreak has been genetically fingerprinted as the typhimurium type, which is one of the most common sources of salmonella food poisoning. Salmonella typhimurium is a problem the occurs year round because it’s found in meat and eggs, unlike some seasonal types that are associated with vegetables.

Peanut Corp. of America said that the peanut butter being recalled is made for distribution to institutions, food service industries and private label food companies. None is sold through retail stores. The peanut butter is sold under the brand name Parnell’s Pride and by the King Nut Co. as King Nut.

Nationally, all the illnesses began between Sept. 3 and Dec. 29, but most were sickened after Oct. 1. Most people develop diarrhea, fever and abdominal cramps 12 to 72 hours after infection. The illness usually lasts four to seven days, and most people recover without treatment.

The peanut butter contamination comes almost two years after ConAgra recalled its Peter Pan brand peanut butter, which contributed to at least 625 salmonella cases in 47 states.
