Tom Chan, 58, was suddenly knocked to the ground and caught under a forklift that had toppled over.

Other employees of the Blue Ridge Radio Repeater Station warehouse attempted to revive Chan.

Crews arrived on the scene approximately 30 minutes after the accident occurred due to the location and terrain near the warehouse.

Chan was unable to survive the injuries he sustained.

It has not yet been reported whether Chan had been driving the machinery or not.

Officials are continuing to investigate the fatal incident.

Patricia Lockwood, 66, was steering a four-wheel mower when she came to a stop to remove some toys from her path.

When she got back on the machinery to finish her neighbor’s lawn, Lockwood accidentally turned it on and it flipped over, landing on top of her.

Police and medical crews arrived on the scene. They pronounced the woman dead at the scene.

The weight of the mower caused Lockwood to asphyxiate.

Police are continuing to investigate the fatal accident.

It was reported to have happened at a construction site at the Chesapeake Energy Corp.

The injured employee suffered non life-threatening injuries and is being treated at a local hospital.

The other employee was unable to survive the injuries he sustained.

The two Mississippi Department of Transportation employees Tyler Kilsby, 25, and Leon Sims, 43, were working on a road when 82-year-old Dorothy Crawley crashed into them. Crawley came to a stop after smashing into a truck.

Kilsby and Sims were unable to survive their injuries. Medical crews arrived and rushed Crawley and the truck driver, 57-year-old Percy Leggett, to a local hospital for treatment. Leggett was released later that day and Crawley has been listed to be in stable condition.

The icy roads may have played a part in the crash. The Lumberton Police are conducting a full investigation into the fatal accident.

The unidentified victim was on Route 28 in Medford when Brian Fountain, 22, crashed into the pedestrian. The victim was rushed to the Massachusetts General Hospital where he was unable to survive his injuries.

Fountain was not injured in the accident and is not facing charges, as of now.

Authorities are conducting a full investigation into the fatal accident.

A female driver was traveling southbound on I-79 when she lost control of her vehicle and crossed the center line, crashing into three northbound vehicles around 6:30 p.m. Wednesday night near mile marker 80.

The southbound female driver and one of the drivers of a northbound car were killed in the accident. At least one of the two other people injured was airlifted to Ruby Memorial Hospital.

The accident is being investigated by the Braxton County Sheriff’s Department.

John Edward Chaney, 65, was driving a 1994 Honda Civic eastbound on state Highway 20, 15 miles east of Colville, when he lost control of the vehicle and drove off the roadway around 6 p.m. The vehicle struck a tree and rolled over, landing on its side.

A passenger in the Civic, Linda Ann Lobdell, 68, was not wearing a seatbelt in the collision and was killed in the accident.

Chaney was taken to Mount Carmel Hospital in Colville where he was treated and released.

Blanche Anderson, 18, was driving a vehicle on Route 16 in Bradley when she lost control and struck a telephone pole.

A passenger in the vehicle, Jessie Anderson, 23, was pronounced dead at the accident scene.

Zachary Watkins, 18, sustained a head injury and broken bones.

Blanche Anderson is charged with DUI causing death, DUI causing bodily injury, aggravated DUI and underage consumption of liquor.

She is being held on $100,000 bond at the Southern Regional Jail.

Andrew J. Sauceda, 28, was riding his Honda XR 650 on Black Quarry Mountain Road in Apple Valley with several other off-duty firemen when he lost control of the vehicle. The bike overturned and Sauceda was thrown from the vehicle over an embankment about 30 feet high around 12:41 p.m. Tuesday, according to California Highway Patrol Officer William Osegueda.

Sauceda, who was wearing a helmet and other protective gear at the time of the accident, was pronounced dead at the scene.

According to police, Newell swerved to miss an alligator that was in the middle of Highway 170 when he struck a bus carrying passengers around 10:45 p.m. Friday.

Newell was pronounced dead at the scene. The alligator was also killed in the accident.

A passenger on the bus, Edward Hubbard, said he was thrown from his seat on the bus when the collision occurred.

The occupants of the bus credit the bus driver for keeping them safe.
